cover the full post code so please
enquire at the surgery to see if you
fall within our boundary.

• You will be given a form to
fill out and bring back to the
surgery
• You will be given an
appointment for a new patient
check with the nursing team –
please bring with you ALL your
medication (or a copy of a
current prescription request)
and a small sample of urine.
DISABLED ACCESS
The practice is accessible to
disabled patients as there are
no steps and all facilities are
located on the ground floor.
